Mobile Vaccination Clinic

Fetter Health Care Network is offering mobile vaccination clinics for individuals based on the South Carolina DHEC guidelines. Currently, we are only vaccinating health care professionals, individuals age 65 and older, mission-critical government employees, long-term care facility residents and staff, and caregiver of medically fragile children. Caregiver must present a signed Pediatric Special Needs Caregiver Attestation form from a medical provider to confirm their status.  When you arrive, follow the instructions provided.  For the privacy of the clinical setting, we ask that you refrain from taking photos or video. Everyone is asked to wear a face covering.  Public restrooms are also not available.

• Personal Photo Identification

• Insurance Card, if applicable

• Provider Attestation form, if applicable

• Employment Identification (Must bring 2 of the following: employer badge, picture of professional license, personalized letter from employer, paycheck stub)
